         In an attempt to make it easier to order a number plate I have made this form.
Sure it  will take a few minutes to fill it out but it should save a lot of  time with emails and all my questions. 
You can fill out as much or as little information as you like.
 Click  Submit at the bottom of the page and the order form will be delivered to me as email.

They cost 20.00 each and the USA postage is included. 
( for  US Postal Service )
Orders of 5 or more is 15.00 each, providing they are similar  pictures.
(you can change the colors numbers and names)

 I include instructions and a self return envelope so  you can send me your payment. 
I accept just about anything.

About Your Handlebars
This is kind of important.
So I know where to put the attachments. Other wise I'll be asking.

Select Type
Pro size, Expert, Junior, Mini, or Cruiser

Do your handlebars have a crossbar?
        

My Bars measure
1- top length of crossbar
 2- the height (from the crossbar down to the bottom)
3- bottom width.

by these measurements I connect the lines to
determine where to put the velcro or zip ties.

Or if your handlebars have a brand name and model etc.
But I do prefer the measurements

Currently there is no extra charge for velcro straps
 and they are attached with aluminum rivets.

Try to choose a shape that goes with your handlebars.
such as, if you have tall bars choose a shape that's more vertical than horizontal.
*Shape #2 fits everything real good.

 

 


Backgrounds
 Choose One
You can have blended colors, a solid color or the flags.

#3 Background Colors
 This is for the background that is behind the numbers.
Usually it's white but some riders have gray or black.
Refer to ABA or NBA rules.

(Solid Color)  if you wanted a solid red, blue
etc. Type in  the color. 
Leave it blank for white behind the numbers

Did you want a number on your plate?
 Some people do not want a number because the plates are
lasting  more than 1 season.
 Also if it is near the end of the season you may want to consider not having a number.
 These numbers are permanent.

Then we can have something on the top and bottom.
Nick names, Logos. Bike shop  etc.
